Writing an Advanced Care Directive 

An advance care plan, formerly known as a “living will”, is a form completed after careful consideration to define and legally record your wishes for health care and end of life wishes. 

This document is particularly important when you are unable to speak for yourself.  These plans are essential for anyone over the age of eighteen as adult laws protect privacy, limiting access to personal information and communication.

The plan format is specific to the state of residence.

My advanced certification through Respecting Choices, a national care plan

training organization, provides me with skills to help you consider your needs,

options and wishes as well as make these critical decisions and record them.

I can help you determine the best health care agent to represent you and

facilitate communication with you and your agent.  

I can assist with  legal completion and registration of the forms, with the option

of including  your directive in a national registry.

We can plan for providing easy access on paper, through your phone, and

using other notification strategies, including medical emergency identifications.
